Prior to participating in any varsity sport at the University of Cincinnati, all incoming student-athletes, including freshman, walk-ons and transfer students, are required to pass an athletic physical exam by a team physician in the Sports Medicine Department. You will need to print, fill out and bring with you the following forms as well as a copy of your insurance card (front and back) and a copy of your immunization records.

Warning Statement and Consent for Treatment
Student-Athlete Physical Form
Authorization for Release of Medical Information
Alcohol, Tobacco and Other Drug Policy
